On-Site Supervisor's Responsibilities
Thank you for agreeing to supervise a Practicum Intern for the semester! Your expertise and guidance will provide new opportunities and learning experiences for the intern. We hope that the intern will be an asset to you as well. As an on-site supervisor you will need to do the following:
1. Make sure the intern signs the daily log book of when he/she worked. The intern is required to work nine hours a week, during the practicum course.
2. Please fill out a biweekly report on the intern's activities - activities performed, skills learned, strengths, and weaknesses. It doesn't have to be in great detail, but needs to reflect the two weeks of work. Please keep a copy of these reports for your notebook. You will need them to complete the final report.
3. You can mail me the reports to:
Dr. Debbie Heroman
